These terms are an agreement between you and [COMPANY LEGAL NAME] (“we,” “us,” “our”). They apply to our website and our online adult ADHD assessment service. By using the website or submitting an assessment, you agree to them. If you do not agree, please do not use the service.

What we provide

We provide online diagnostic evaluations for ADHD in adults. The service includes an online questionnaire, review of your responses by a state-licensed clinical psychologist within 48 hours, a 30-minute video consultation, a DSM-5-TR diagnostic decision, a written report, and a superbill.

We provide diagnosis only. We do not prescribe medication, provide therapy, or offer ongoing treatment. Our report is intended to help you and your treating providers make decisions. Whether a prescriber or other provider accepts it is their decision.

We do not guarantee any particular result. You may complete the evaluation and not meet criteria for ADHD. Fees pay for the clinician’s time and professional judgment, not for a specific outcome.

Who may use the service

To use the service you must:

  • Be at least 18 years old
  • Be physically located in a US state where our reviewing clinician is licensed at the time of your video appointment
  • Be completing the assessment for yourself, not on behalf of someone else
  • Provide accurate and complete information

We will ask you to confirm your location before your appointment. If you are not in an eligible state at the time of the appointment, we cannot proceed and the refund policy will apply.

Not an emergency service

Our service is not for emergencies and is not monitored around the clock. If you are in crisis, thinking about harming yourself or someone else, or need urgent help, call or text 988 to reach the Suicide and Crisis Lifeline, call 911, or go to the nearest emergency room. Do not use the assessment or our contact forms to report an emergency.

We do not require you to create a password-protected account. Instead, we may email you a secure link that lets you resume an unfinished assessment or access your results. Keep that link private. Anyone who has it may be able to see your information. Tell us right away at [CONTACT EMAIL] if you think a link has been shared or compromised.

Payments

Our fees are:

  • An evaluation fee, charged when you submit your assessment
  • A consultation fee, charged 48 hours before your scheduled video appointment

Current fees are shown on the pricing page. Payments are processed by Stripe. We do not bill insurance and are out of network with all plans. We provide a superbill you may submit to your insurer for possible out-of-network reimbursement, but we cannot guarantee reimbursement. HSA and FSA cards are accepted.

Cancellation, rescheduling, and refunds are covered in our Refund policy, which is part of these terms.

Your responsibilities

You agree to:

  • Answer honestly and completely
  • Attend your appointment on time from a private location with a working camera, microphone, and internet connection
  • Use the service only for yourself and for lawful purposes
  • Not copy, scrape, reverse engineer, or interfere with the website or assessment
  • Not record your consultation without the clinician’s agreement

We may decline or discontinue service if you provide false information, behave abusively toward staff or clinicians, or violate these terms.

Our service is delivered by telehealth. By using it, you agree to our Telehealth consent, Privacy policy, and Notice of privacy practices, which are all part of these terms. We comply with HIPAA in handling your health information.

Intellectual property

The website, its text, design, assessment structure, and reports are owned by us or our licensors and are protected by copyright and other laws. You may use your own report for your personal health care. You may not reproduce or republish our materials for other purposes without written permission.

Disclaimers

The website’s educational content is for general information and is not a substitute for individual medical advice. Our clinicians exercise professional judgment based on the information you provide, and a diagnosis can be wrong if that information is incomplete or inaccurate. Except as required by law, the service is provided “as is” without warranties of any kind, express or implied.

Limitation of liability

To the fullest extent permitted by law, we are not liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits or data, arising from your use of the service.

Our total liability for any claim relating to the service will not exceed the amount you paid us in the 12 months before the claim arose. Some states do not allow these limits, so they may not apply to you in full. Nothing in these terms limits liability for professional negligence where the law does not allow it to be limited.

Changes to the service and these terms

We may change the website, fees, or these terms. Changes to fees will not affect an assessment you have already paid for. We will post updated terms with a new effective date, and continued use after that date means you accept them.

Governing law and disputes
