ADHD assessment cost and insurance

The complete online ADHD evaluation costs $388 with no subscription. You pay $189 when you submit the questionnaire and $199 for the psychologist appointment, charged 48 hours before it. We do not bill insurance, but we provide a superbill for out-of-network claims and accept HSA and FSA cards.

Clinically reviewed · Updated October 2, 2026

Cost is one of the main reasons adults put off an ADHD evaluation. A traditional in-person assessment by a psychologist typically costs between $1,500 and $3,000, sometimes more, and often involves a wait of several months. We have set our fees so the price is clear before you begin and so there are no surprises later.

What the evaluation costs

The full evaluation is $388, paid in two parts. There is no subscription, no membership and nothing else to buy.

Online evaluation: $189

This fee covers the questionnaire portion and the clinical review that follows:

  • All screening instruments, including the ASRS v1.1 and screeners for anxiety, depression, PTSD, well-being, panic and substance use
  • The childhood history and intake sections
  • Review of your responses by a state-licensed clinical psychologist within 48 hours

The $189 is charged when you submit your completed evaluation.

Diagnostic appointment: $199

This fee covers the clinical part of the diagnosis:

  • A 30-minute video appointment with a licensed clinical psychologist
  • A structured clinical interview using DSM-5-TR criteria
  • Your written diagnostic report
  • A superbill with CPT and diagnostic codes

The $199 is charged 48 hours before your scheduled appointment. You can reschedule or cancel free of charge up to 48 hours before the appointment. Full terms are in our refund policy.

How this compares to a traditional evaluation

In-person ADHD evaluations for adults commonly run $1,500 to $3,000 and can go higher in large cities. Part of that cost reflects office space and administrative staff.

Part of it reflects long testing batteries, which can be useful in complex cases but are not required for an ADHD diagnosis under the DSM-5-TR. ADHD is diagnosed through a clinical interview, a symptom history and validated rating scales.

Our process keeps the parts that drive accuracy, the validated instruments and the psychologist interview, and drops the overhead. Research on telehealth assessment supports this approach; a systematic review found that test results gathered by videoconference did not differ significantly from in-person administration (Brearly et al., 2017). We go into the evidence on our accuracy of online testing page.

Some primary care doctors will do a brief ADHD screen during a regular visit, and insurance may cover it. That is a reasonable first step, but it usually relies on a single questionnaire and a short conversation rather than a full diagnostic evaluation with a written report.

Insurance and superbills

We are an out-of-network provider. We do not bill insurance, and we are not in any insurer’s network. This keeps our fees low and means you never need a referral or prior authorization to begin.

After your evaluation we provide a superbill. This is an itemized receipt that lists the services provided, the CPT codes for psychological evaluation, the diagnostic codes from your report, the dates of service and our provider information. It contains what your insurer needs to process an out-of-network claim.

Whether you are reimbursed, and how much, depends on your plan:

  • PPO and POS plans often include out-of-network mental health benefits. Reimbursement is typically a percentage of the fee after you meet your out-of-network deductible.
  • HMO and EPO plans usually do not reimburse out-of-network care.
  • High-deductible plans may apply the full amount to your deductible.

The quickest way to find out is to call the member services number on your insurance card before you start. Ask whether your plan has out-of-network benefits for psychological evaluation, what your out-of-network deductible is and how much of it you have met, and what percentage the plan reimburses. Ask how to submit a claim too; most insurers accept uploads through a member portal.

We cannot guarantee reimbursement, and the claim is yours to file. We are happy to answer questions about the superbill itself through our contact page.

HSA and FSA

A diagnostic evaluation by a licensed psychologist is a qualified medical expense, so you can pay both fees with a Health Savings Account or Flexible Spending Account card. Because these accounts use pre-tax money, this lowers your real cost. Keep your payment receipt and superbill in case your plan administrator asks for documentation.

We also accept major credit and debit cards.

What is not included

We diagnose but do not prescribe. If your report leads to a conversation about medication, that happens with your own primary care doctor or a psychiatrist, and their fees are separate. Likewise, ongoing therapy or coaching is arranged with providers of your choice. Our treatment options overview describes what usually comes next and what it tends to cost.

Refunds

Fees are generally final once charged. We do not refund the evaluation fee if the outcome is not an ADHD diagnosis, because the clinical work is the same regardless of the result. If we have to cancel your appointment, we will reschedule it or refund the appointment fee. If you cannot attend, let us know at least 24 hours in advance so the appointment can be moved. The complete terms are on our refund policy page.

Common questions

When am I charged?

The $189 evaluation fee is charged when you submit the online questionnaire. The $199 appointment fee is charged 48 hours before your scheduled video appointment. There are no other fees and no subscription.

Do you take insurance?

We do not bill insurance or join insurance networks. After your evaluation we give you a superbill, an itemized receipt with CPT and diagnostic codes, which you can submit to your insurer for out-of-network reimbursement if your plan offers it.

Can I pay with an HSA or FSA card?

Yes. A psychological evaluation is a qualified medical expense, so you can pay either fee with a Health Savings Account or Flexible Spending Account card. Keep your receipt and superbill as documentation.

Why is this so much cheaper than an in-person evaluation?

Traditional evaluations often involve several office visits and lengthy testing batteries that the DSM-5-TR does not require for an ADHD diagnosis. Our process uses validated rating scales plus a structured interview, delivered online with no office overhead, and a psychologist still makes every diagnosis.

What is your refund policy?

Fees are generally non-refundable once charged, including when the outcome is not an ADHD diagnosis. If we cancel an appointment, we reschedule or refund it. Full details are on our refund policy page.
